药品审评检查机构质量管理体系建设实践探索

目的:贯彻国务院强化质量管理、落实全国药监部门完善自身质量管理体系工作部署,梳理山东省药品审评检查机构质量管理体系建设情况,总结经验、查找不足,加强审评检查质量管理。方法:调研药品审评检查机构导入质量管理体系的现实意义,剖析山东省药品审评检查机构质量管理体系建设的实践经验,思考存在的问题。结果:行之有效的质量管理体系对药品审评检查机构规范化建设和药品审评检查发展起到了积极的推动作用。结论:药品审评检查机构推进质量管理体系建设,对规范自身建设、提高工作质量、降低业务风险、服务医药产业发展具有重要作用,也是必然要求,药品审评检查机构质量管理体系需要持续改进,逐渐与国际接轨。     

Comparison of microtransplantation,chemotherapy and allogeneic transplantation in post-remission therapy for Philadelphia chromosome-positive acute lymphoblastic leukemia

Patients with Philadelphia chromosome-positive acute lymphoblastic leukemia (Ph⁺ ALL) have poor prognosis, and the efficacy of chemotherapy plus tyrosine kinase inhibitors (TKIs) followed by mismatched donor stem cell infusion (microtransplantation, MST) has not been determined. We retrospectively summarized 45 patients including 11 undergoing MST with TKIs, 17 receiving allogeneic transplant and 17 undergoing chemotherapy with TKIs. Improved 4-year overall survival rate was observed in the MST group (91%) compared with either transplant group (31%, P = .005) or chemotherapy group (36%, P = .013). The MST group also had higher 2-year and 4-year leukemia-free survival rates (91% and 72%, respectively) compared with either transplant group (33%, P = .005 and 33%, P = .021, respectively) or chemotherapy group (41%, P = .017 and 31%, P = .023, respectively). 2-year and 4-year cumulative incidences of hematologic relapse were lower in the MST group (9% and 28%, respectively) compared with those in the chemotherapy group (56%, P = .025 and 67%, P = .034, respectively). In patients undergoing MST, donor microchimerism was detected (1.07 × 10^-5 to 6.6 × 10^-4 copies from 9 to 1499 days) in 7 patients, and donor/patient-derived HLA*0201/2402⁺WT1⁺CD8⁺ T cells were found from 0.05% to 0.67% in 6 patients. MST may provide a favorable treatment for patients with Ph⁺ ALL.     

circRNA在肝癌中的研究进展

circRNA是一种新的内源性RNA，广泛存在于真核细胞中，参与转录或转录后基因表达调控，大部分属非编码RNA，是近几年RNA领域中的研究热点。circRNA主要通过拮抗miRNA来发挥“海绵作用”进而调控靶基因表达，与多种疾病有着密切联系，特别是在肿瘤中发挥重要作用。近年研究发现，circRNA对肝细胞肝癌（hepatocellular carcinoma，HCC）的发生、发展及转移具有重要调控作用。该文就circRNA与肝细胞肝癌关系的研究进展作一阐述。     

住院医师规范化培训中引入Mini-CEX的实践

目的　观察对住院医师规范化培训部分学员使用Mini-CEX进行临床教学的效果。方法选取我院2015级住培学员40名，随机分为对照组和实验组，每组20人。2015年10月至2018年4月，对照组采用传统教学；实验组在传统教学基础上结合Mini-CEX活动，实验组每人在每个轮转科室至少进行2次Mini-CEX。毕业时应用改良的Mini-CEX评分量表、病例答辩和OSCE三种考核方法对两组学员进行测评。针对实施过Mini-CEX的教师和学员分别发放问卷，进行满意度调查。采用SPSS 18.0对数据进行统计分析，组间比较采用独立样本t检验，组内前后比较采用配对样本t检验。结果 Mini-CEX测评显示，两组学员各项临床能力毕业成绩均高于入学成绩，实验组学员毕业成绩优于对照组，实验组学员成绩提高分数大于对照组提高分数，差异均有统计学意义（P＜0.05）。病例答辩显示，实验组学员平均成绩为（81.16±3.75）分，优于对照组的（70.13±3.88）分，差异有统计学意义（t= -9.140，P=0.000）。OSCE考核显示，实验组平均总分为（96.300±4.681），优于对照组的（91.775±3.227）分，差异有统计学意义（t=-3.559，P=0.001）。共发放调查问卷80份，其中教师40份、学员40份，结果显示教师满意率达95.0%、学员满意率达92.5%。结论　Mini-CEX应用于住院医师规范化培训临床带教中，有利于提高学员的临床思维、医患沟通和人文关怀能力。     

重组戊型肝炎病毒P179-海藻酸钠/壳聚糖微球混悬制剂的制备及其免疫效果

目的 制备吸附重组戊型肝炎病毒(hepatitis E virus,HEV)P179抗原的海藻酸钠/壳聚糖微球混悬制剂,并观察其免疫效果。方法 乳化法制备海藻酸钠/壳聚糖微球,采用不同海藻酸钠浓度、混合乳化剂添加量、混合乳化剂亲水亲油平衡值设计正交试验,确定最佳制备工艺参数。将重组HEV P179抗原吸附在最佳工艺制备的微球表面,制备吸附重组HEV P179的海藻酸钠/壳聚糖微球混悬制剂,皮下多点免疫BALB/c小鼠,测定其诱导小鼠产生特异性IgG抗体的能力,与同剂量含弗氏佐剂的重组HEV P179免疫制剂对比。结果 经正交试验确定乳化最佳工艺参数为:海藻酸钠质量分数1%、混合乳化剂体积分数2%,混合乳化剂亲水亲油平衡值4。制备的微球平均粒径为6.73 μm(分布范围3.90~9.10 μm,方差1.78 μm),形态较为均一,透射电镜观察结果与双层球体及内部疏松多孔的微球结构特点相符。用此条件制备抗原质量浓度为500 μg/ml的混悬制剂,微球对抗原的吸附率为90.64%。免疫效果观察试验结果表明,皮下多点免疫试验中微球制剂诱导特异性IgG抗体的能力优于含弗氏佐剂抗原。结论 成功制备了吸附重组HEV P179的海藻酸钠/壳聚糖微球混悬制剂,且诱导产生特异性IgG抗体的能力优于含弗氏佐剂抗原,为其在新型实验动物超敏制剂中的应用奠定了基础。     

欧盟草药药品监管模式对中国中药注册管理的启示＊

对药品实行注册管理制度是确保公众健康的全球通用的监管措施。中药作为承载中医药理论与用药实践的主要载体，对中药产品的注册管理又具有其特殊性。2017年正式实施的《中医药法》提出了建立符合中医药特点管理制度的发展方针，研究制定配套文件或修订现行规章以适应衔接其立法精神是当前重要的现实问题之一。欧盟在植物药监管领域取得了良好成效，本文旨在系统分析欧盟对其注册管理的法规体系，深入剖析其监管特点和对中国中药注册管理的启示，以期为当前《中医药法》背景下中药监管制度的改革提供参考。     

The predictive value of crescents in adult Henoch-Sch?lein purpura nephritis

Objective To study the renal prognosis with the type and proportion of crescentic in adult Henoch Schonlein purpura nephritis (HSPN). Methods A total of 275 HSPN cases diagnosed in the First Affiliated Hospital of Zhejiang University were retrospectively analyzed. According to the pathological results, they were divided into four groups: 99 patients in none crescent group (NC), 35 patients in segmental crescents group (SC), 122 patients with circumferential crescent ＜25% (C1), and 19 patients with circumferential crescent≥25% (C2). Renal prognostic events were defined as estimated glomerular filtration rate (eGFR) decreased by 30% over baseline within 2 years, doubling of serum creatinine or end-stage renal disease during follow-up. Kaplan-Meier survival analysis was used to compare the renal survival rate of each group. Univariate and multivariate Cox regression model was used to recognize the risk factor of poor renal outcome. Results There was no significant difference in age, extra renal organ performance and mean arterial pressure among groups. Among NC group, SC group, C1 group and C2 group, difference in serum creatinine (P=0.001), eGFR (P=0.003) and proteinuria levels (P＜0.001) were statistically significant. There was no significant difference in the ratio of global sclerosis, mesangial hypercellularity and interstitial inflammation/fibrosis among the groups. The patients were followed up for 86(58, 116) months. The renal survival rates of NC group, SC group, C1 group and C2 group were 96%, 100%, 83.6% and 68.4% respectively. Kaplan-meier survival analysis showed significant differences (Log Rank=23.24, P＜0.001). Cox multivariate regression analysis indicated that presence of circumferential crescent (HR=3.59, 95%CI 1.34-9.62, P=0.008) and low eGFR (HR=0.979, 95%CI 0.968-0.989, P＜0.001) were independent prognostic factors. Conclusion The presence of circumferential crescent and low eGFR level are independent risk factors for poor renal prognosis in HSPN patients.
